Procedure for Applying for Adaptive Needs

1. Complete the following items:
• Application for Disability Services
• Permission to Release Information
2. Attach recent documentation from a qualified professional stating the nature of the disability and necessary accommodations (see Disability Guidelines form).
3. Submit the complete packet to the Dean of Students in Malone Hall, Room 100.
