REQUEST FOR TRANSCRIPT

For convenience, prospective graduate students should complete the transcript request form on line for each university or college previously attended, print off the forms, sign each one and mail to each of the colleges.  Be sure to include the proper amount of money to cover the transcript cost.  If student is unsure about the cost, please call the college and check on the cost before mailing the form.  This will prevent delays in having the transcripts mailed.
